要旨： 硬化性胆管炎はいくつかの病態で発生するが，その中で原発性硬化性胆管炎が代表的であり，これまで欧米を中心に研究されてきた．進行性であり，肝移植を除けば確立された治療法はない．最近，自己免疫性膵炎やIgG4に関連した硬化性胆管炎の存在がわが国から発信され，国際的にも注目されている．この硬化性胆管炎では，密なリンパ球，形質細胞の浸潤があり，特にIgG4陽性形質細胞の浸潤が高度で，ステロイド治療が著効する．さらに，臨床的に胆管癌と誤診され，不必要な外科的切除が行われた例もある．現在，これら硬化性胆管炎の鑑別診断が重要となりつつある．病理所見を中心に，硬化性胆管炎の診断と問題点について述べる．
