麻酔科医の視点で見た挿管 tracheal intubation
sniffing positionにする。左手で開口して、右示指と母指をできるだけ臼歯の奥に入れる。右示指で上顎を頭側に上げつつ母指で下顎を尾側へ押し開口する（closs finger)。喉頭鏡を右側に斜めに挿入して舌を左へ圧排する。口唇を、喉頭鏡と歯の間に挟んで損傷しないように注意。喉頭展開して声門を確認したら、自分の視野を遮らない様に、右側からﾁｭｰﾌﾞを挿入。右口角を助手に牽引してもらって視野を作っても良い。声門を通過したらスタイレットを抜去。ﾁｭｰﾌﾞを左手に持ち替える。深さが変わらないように、指を口角に、小指球は左頬部に乗せておく。胸部の挙上、呼気でﾁｭｰﾌﾞが曇るのを確認。五点聴診。CO2の検出を確認。テープ固定。
