中嶋 匡1), 西村 裕之1), 西原 賢太郎2), 浮田 透2), 辻 雅夫2), 三宅 裕治2), 大村 武久2), 立花 久大3) 要旨: 症例は68歳女性．2006年11月21日右手の動かしにくさを自覚し，以後徐々に症状増悪した．11月23日には構音障害と運動性失語が出現した．11月24日右上肢のけいれん後右片麻痺が出現し当院へ入院した．入院時，意識障害，全失語，軽度右片麻痺を認めた．入院当日の頭部MRIは，拡散強調画像およびFLAIR画像で左前頭葉皮質にリボン状に高信号を認めた．緩徐進行性の経過から，seizureを伴った血栓性脳梗塞と診断し，抗てんかん薬投与と抗血小板療法を行った．入院後物品呼称や名前を言うことが可能となり，右下肢麻痺は消失した．右上肢麻痺も徐々に改善し，失語症と共に26日には消失した．以上より本例をfocal inhibitory seizureと診断した．本
