自己免疫脳炎の代表的疾患である抗NMDAR脳炎は，精神症状，記銘力障害，痙攣発作，運動異常症，意識障害，中枢性低換気などを呈する若年女性にみられる脳炎です．急性期から積極的な免疫療法を行うことが重要で，第１選択療法でうまくいかないとき，速やかにリツキシマブなどの第２選択療法に踏み切れるかが重要です．ただし現在の治療は，広範な免疫抑制ないし非選択的抗体除去ですので，限界があり副作用も問題になります． 今回，Cell誌に，ドイツからCAR-T細胞療法でNMDAR抗体を作るB細胞を選択的に除去するという研究が報告されました！ CAR-T細胞療法は脳神経内科では馴染みがありませんが，急性リンパ性白血病や悪性リンパ腫といった血液がんに大きな進歩をもたらした治療です．少し解説すると，近年，T細胞免疫療法において，キメラ抗原受容体（chimeric antigen receptor: CAR）をもつ遺伝

標題に関してとても分かりやすい図を掲載した総説がありました．以下，まとめです． １）視神経（図１） MOGAD ではガドリニウム造影病変（Gad+）は前方に長く，視神経鞘や眼窩脂肪の異常信号や乳頭浮腫を伴う．NMOSDではGad+は視神経交叉に認められる．MSではGad+は短く，多くの場合，中ほどに認める．Gad+は通常経過観察ですべて消失する． ２）脊髄（図２） MOGADでは脊髄のT2病変は長大で多発し，脊髄円錐も傷害し，軸位断ではH型を呈する．NMOSDでは単発性で長大で，軸位断で中央に円形を呈する．MSでは多発性，短く，辺縁に認められる．経過観察では MOGADの病変はしばしば消失するが，NMOSDとMSでは残存し，MSでは新たな病変が生じる． ３）大脳病変 ①MOGAD（図３） FLAIR異常信号を，橋，中小脳脚，視床，大脳白質，皮質に認め，Gad+は軟膜・くも膜に認める．6～1

自己免疫性脳炎の研究は非常に大きな進歩を遂げています．抗神経抗体は細胞表面抗原と細胞内抗原を認識する抗体に大別されますが，前者は治療可能性が高いことから見逃さないことが重要です．さらに後者でもGFAP抗体やKLHL11抗体のように比較的免疫療法が奏効する脳炎もあります．しかし急速に自己抗体が増加し，その臨床像が明らかになったためフォローが難しく，症候から自己抗体を推定することが難しくなっています．今回，Oxford大学のグループから発表された総説は，治療可能で見逃したくない自己抗体について解説したもので，非常に有用です．とくに各抗体が呈しうる臨床症候の頻度をヒートマップで，希少または不明（0＝青）から一般的（4＝赤）まで示したFigureは役に立ちます． 上段の細胞表面抗原は受容体が多いため，痙攣や意識障害，そして記憶障害が多く認められることが分かります．運動異常症（hyperkineti
